COMET P/2004 V5 = P/2003 YM_159 (LINEAR-HILL)
This comet was discovered during the course of normal asteroid search
on the night of 10 Nov., 2004. Details about it can be seen at:

Below is the discovery image from one of the standard search fields using 
the CSS schmidt. In this field the two pieces of the comet can be seen in 
the circles.
